Gentlemen:

\JI am enclosing an IBM Family Medical Plan Application.  Since it
has been several years since I have submitted any claims and the methods of
hanldling these claims has been changed, I am not too sure as to how to
supply the desired information or as to just what is covered under the
plan. 

There is also another complication.  My wife and I now reside in The Sequoias,
a retirement community where there is a monthly charge which includes complete
medical care. It is my understanding that this does not make us inelgible
for IBM benefits and that the correct procedure is to submit the Medicare
reports as evidence of the expenses which they actually paid on my account.

Will you please advise me as to the way this matter should be handled
for subsequent years.\.
